The Rexdale Medical Centre is pleased to offer pain and symptom management at home for those patients who suffer from the late stages of many diseases such as Cancer, Dementia, Kidney failure, Heart Failure or Emphysema. Rexdale Medical Centre has three physicians that are trained and care for palliative care patients in local community. Dr. David Caplan, Dr. Joyce Cheung and Dr. Tim Chung are fully trained Palliative Medicine Doctors who provide these services for patients in need. These physicians work closely with the Central and Central West Community Care Organizations, who follow patients regularly at home in the Rexdale area of Etobicoke. Referrals for this service can be provided by the Primary Care Physician who is currently managing the patient or the supervisor from the Community Care Access Team (CCAC)

Who to Call and When
For support with pain and symptoms Days, Evenings, After Hours, Weekends, and Holidays:
Step 1. Please call your Nursing Agency to ask for an extra nursing visit. Your nurse will call the Palliative Physician if this is needed.
Step 2. If you do not know the phone number of your Nursing Agency then call CCAC at Tel: 310-2222 [TC1] to ask for an extra nursing visit. (Please note that this phone number has only 7 numbers).
Step 3. If you are unable to obtain satisfactory response, please call your Palliative Physician at 647-556-5516 to leave a voice message and they will get back to you within a business day.
For family members: if you family member has passed away:
Please contact your Nursing Agency. THERE IS NO NEED TO CONTACT 911.
A nurse will come to pronounce your family member. They will then contact your funeral home and your palliative care physician to make arrangements for the body to be transferred.
